Office Policies

  • If you have an emergency and are unable to reach the doctor, please call 911 or go to the nearest hospital.

  • Co-payments are due at the time of the visit. There will be a $5 charge for billing for co-pays (it costs time and money to bill). For co-pays less than $20, cash or checks are only accepted.

  • We will give refills on most medications up to 6 months after the appointment which the medication was prescribed. Medication refills should be electronically sent by the pharmacy or the pharmacy should call the office.

  • No medication refills, referrals, or appointments will be given if your account is not completely cleared of all charges.

  • If there is any insurance changes, it is your responsibility to give us that information. Some insurances only have 90 billing days time after which they will not pay, thus making you responsible.

  • Although we see patients by appointments, we will see you in the office any day we are open if you are sick and need immediate attention. For this reason, sometimes we get behind on the schedule.

  • There will be no controlled substances prescriptions to be given or refilled (Xanax, Valium, Percocet, etc.) without being seen in the office. This is the law by the state CDC.

  • If you have any tests or biopsies done, make sure that you go over the results with the doctor. Do not assume that if you do not hear anything from our office, everything is okay. It is possible that the reports were lost in the mail, etc. or were never seen by the doctor.

  • Abnormal labs and other results cannot be discussed over the phone. We do not know who is calling on the phone. You can pick up a copy of your results. Due to privacy laws, we cannot mail or fax results to you directly.

  • Filling our disability forms takes time. There will be a $10 charge for filling out disability and other physical forms.

  • Unless it is an ongoing treatment, new referrals will no be made unless the doctor sees you and deems it necessary for you to see the specialist.

  • For all new patients make sure all new medical records are forwarded to our office.

  • If you have any problems with the staff or office policy, please let us know so that we may rectify the problem.
